At COMPANY, our goal is to have everyone doing their best work. We have observed that elements of your performance are not meeting the expectations we have for your role. This Performance Improvement Plan (the “Plan”) details the concerns regarding your performance as well as the factors that would constitute acceptable performance of your job duties. The Plan requires you to articulate a set of action items to bring your performance to an acceptable level, including identifying resources you believe will help in this effort. Finally, the Plan lays out next steps and timing requirements for you to follow. Please read the Plan carefully and respond accordingly.

Next Steps
You will complete the above “Plan of Action” and “Resources Needed” sections of the Plan, sign where indicated below, and deliver it to me by 10 a.m. PT on DATE. Your manager, team executive, and People Partner will review your responses and, if the company finds them acceptable in terms of achieving the job performance requirements outlined in this Plan, we will advise you accordingly. If you do not provide the above by 10 AM PT on DATE, we will assume that you are not interested in moving forward with this Plan or continuing your employment with COMPANY and will deem this your resignation from COMPANY effective immediately.
We will meet in two weeks to discuss your progress toward performance improvement and to share feedback with you. In 4 weeks we will meet again to assess your overall performance.
If by DATE it is determined that you are not fulfilling the expectations of your role we will consider additional action which may include the termination of your employment.
Your People Partner will meet with you weekly during your PIP to conduct check-ins and answer any questions you may have.
We expect to see marked improvement in your performance over the course of this evaluation period. If at any time your performance is not progressing as expected the company may take additional action, which may include the termination of your employment.
